Duties & Responsibilities: The Child/Adolescent Care Coordinator is part of a grant-funded program initiative in Richmond County, the Care Management Entity (CME). The focus of the program is provision of intensive service coordination and family support services ("Wraparound Services")to youth who are at risk of being referred to an out-of-home placement. The services are not authorized under the Medicaid Rehabilitation Option (MRO), but are in conjunction with therapy. The goal is to enhance the family and youth's well-being by keeping the youth in the family home and community while promoting self-advocacy.
The Care Coordinator is one of a two-member team of para-professionals which assist the youth and family in meeting mental health, academic and social needs. The Care Coordinator is responsible for the following:
---Facilitates child and family team meetings that focus on identification of strengths and areas for development.
---Using results of these meetings, creates an action plan for the youth and family.
---Regularly reviews and revises the action plan as the youth and family make progress or as needs change.
---Ensures youth and family crisis and community safety responses.
---Assists the youth and family in meeting identified behavioral health, social and other support needs.
---Assists the youth and family with transition planning, including academic planning, adult transitions, etc.
Minimum Training & Experience: High school diploma or GED AND three years experience in a social services related position (case management, outreach, prevention/education).
Bachelor's degree in a social services related field (social work, psychology, counseling, sociology) AND One year experience in a social services related position (case management, outreach, prevention/education).
One year at the Community & Social Services Tech, Working Level.
Agency Specific Qualifications and/or Preferred Qualifications: Bachelors degree and at least two years experience in a case management role with children and families. Experience working in inter-agency collaborative projects. Experience working independently.
