Job Description:

Job Summary: 

The Community Coordinator participates in the planning, implementation, and case management of individual reentry plans and engages stakeholders, including supervision caseload, their families and significant others, human service agencies, and volunteer and faith-based organizations in the process. The Community Coordinator facilitates the seamless and successful execution of program goals and initiatives in the local communities; ensures stakeholder knowledge and solicits program support and sponsorship from diverse groups within the community. Serves as a liaison between the local community, support staff, and affiliated agencies to ensure the flawless execution of program logistics and client success.


Responsibilities/Duties:

  • This position is currently designated as a hybrid (50% in-office and 50% working in the community to build resource capacity and/or virtual work) position. This position is full-time (40 hours per week), requiring travel within the assigned circuit.  

  • Manages assigned caseload by providing intensive case management and facilitating linkages and communication with resource and service providers 

  • Builds and maintains relationships with community stakeholders  

  • Works in the community to expand capacity with services such as housing, employment, substance abuse, mental health, transportation, etc. 

  • Address the risks and needs and build on the identified strengths of each individual.  

  • Coordinates and communicates DCS reentry goals and initiatives; facilitates the process of open communication and effective coordination to ensure community input is captured, and expectations for implementation are clearly defined. 

  • Coordinates meetings, focus groups, and other local events to ensure effective two-way communication channels are established and maintained at all levels within the community. 

  • Educates chiefs and CSOs on reentry services available.  Obtains feedback from CSOs as to their needs and how to improve reentry processes.  Attend staff meetings with respective offices and inform them of upcoming reentry events. 
     

Minimum Qualifications: 

Bachelor's degree in a related field from an accredited college or university and four (4) years of professional job-related experience, which includes one (1) year in a lead worker, team leader, or supervisory role; or two (2) years of experience at the lower level Social Services Program Consultant 2 (SSP041) or position equivalent.


Preferred Qualifications: 

Preference will be given to applicants who, in addition to meeting the minimum qualifications above, also possess three or more of the following: 

Two years or more of experience in program monitoring and compliance

Two years or more of partnering with community and civic groups, community and civic leaders, agency leaders, public officials, and the public to gain support for program initiatives. 

Two years or more with strategic planning, implementation, community organizing, etc.

Two years or more of experience working in a human service delivery area.
